About the Job
Accelerated Home Health Care is seeking a licensed and compassionate Speech-Language Pathologist (SLP) to provide in-home therapy services under physician orders and in coordination with the care team.
Key Responsibilities:
- Conduct evaluations and develop individualized treatment plans.
- Implement evidence-based speech and language interventions.
- Educate patients and caregivers on strategies and exercises.
- Document patient progress and communicate with the interdisciplinary team.
- Follow physician orders and all applicable regulations.
Minimum Certification & Licensing Requirements:
- Master’s degree in Speech-Language Pathology.
- CCC-SLP certification and state licensure.
- Completion of agency onboarding/credentialing.
- Up-to-date TB test and reliable transportation.
Preferred Qualifications:
- Home health experience preferred.
- Strong communication and clinical skills.
- Ability to work independently in a home setting.
Working Conditions:
- Travel to patient homes required.
- Flexible scheduling including occasional weekends.
- Physical demands include standing, bending, and assisting patients.
