Accelerated Home Health Care is hiring a Medical Social Worker (MSW) to provide in-home psychosocial support to patients and families.
Key Responsibilities:
- Conduct assessments of psychosocial needs.
- Provide counseling and emotional support.
- Connect patients and families to community resources.
- Document interventions and communicate with the care team.
- Follow care plans and regulatory requirements.
Minimum Certification & Licensing Requirements:
- MSW degree and state licensure.
- Completion of agency onboarding/credentialing.
- Up-to-date TB test and reliable transportation.
Preferred Qualifications:
- Experience in home health or hospital settings.
- Strong interpersonal and problem-solving skills.
- Ability to work independently in a home-based setting.
Working Conditions:
- In-home visits with travel between patients.
- Flexible schedule, including occasional weekends.
- Must be able to handle in-home work safely and independently.
