About the Job
Accelerated Home Health Care is seeking a licensed and compassionate Occupational Therapist (OT) to provide in-home occupational therapy services to patients under physician orders and in coordination with the interdisciplinary care team.
Key Responsibilities:
- Provide in-home occupational therapy services, including Start of Care (SOC) and revisit visits.
- Complete patient evaluations and establish individualized treatment goals.
- Implement evidence-based interventions to improve independence in activities of daily living.
- Educate patients and caregivers on adaptive techniques, safety strategies, and use of equipment.
- Accurately document evaluations, treatment notes, and progress reports.
- Communicate patient status and progress with the care team.
- Follow physician orders, care plans, and all applicable state, federal, and agency regulations.
Minimum Certification & Licensing Requirements:
- Active Occupational Therapist (OT) license in the State of Michigan.
- Completion of required state and agency onboarding/credentialing.
- Up-to-date TB test.
- Valid driver’s license and reliable transportation.
Preferred Qualifications:
- Home health experience preferred (not required).
- Strong clinical judgment and communication skills.
- Ability to work independently in a home-based setting.
- Organized, reliable, and patient-focused.
Working Conditions:
- Services provided in patients’ homes; travel between visits required.
- Flexible scheduling, including daytime hours and occasional weekends.
- Physical demands include standing, bending, lifting, and assisting patients as needed.
